About the NRCME Exam
What is the NRCME exam?
The NRCME (National Registry of Certified Medical Examiners) exam is a certification test required by the FMCSA for healthcare providers who want to perform DOT physical examinations for commercial motor vehicle drivers. It consists of 120 multiple-choice questions covering 14 categories. Learn more about the NRCME exam.
Who can take the NRCME exam?
Licensed MDs, DOs, DCs, PAs, APRNs, and NPs with an active, unrestricted medical license can take the exam. You must also complete an FMCSA-accredited training program before sitting for the test. See the full step-by-step process.
How many questions are on the NRCME exam?
The exam has 120 multiple-choice questions. You must demonstrate competency across all 14 tested categories, not just achieve an overall passing score.
What score do I need to pass?
The NRCME exam does not use a fixed percentage passing score. Instead, the passing standard is determined by psychometric analysis and requires you to demonstrate competency across all tested categories. This means there is no single number to target—you need a solid understanding of all 14 exam areas.
What is the pass rate for the NRCME exam?
While the FMCSA does not publish official statistics, the first-attempt pass rate is estimated at 70–80%. About 1 in 4 to 1 in 5 candidates do not pass on their first try. Read our detailed analysis of the pass rate.
Can I retake the NRCME exam if I fail?
Yes. If you don’t pass on your first attempt, you can retake the exam. There may be waiting periods between attempts, and you’ll need to pay the testing fee again. After multiple failures, the FMCSA may require additional training before your next attempt.
How long is my NRCME certification valid?
Your NRCME certification is valid for 10 years. You must complete recertification training and pass the exam again before it expires. Read our recertification guide.
How long should I study for the NRCME exam?
Most candidates who pass on their first attempt study for 2 to 4 weeks, spending 1–2 hours per day. We recommend reading the FMCSA Medical Examiner Handbook in the first week, then spending the remaining weeks on practice questions and full-length exams. See our complete study guide.
